If psychological assessment has been recommended by the school, please ask the school staff to fill out the form below. Without the form, we will not be able to schedule an appointment, so please complete the form when you are ready. Please attach the document with the English form. 

☆心理アセスメントをご希望の方（インターナショナルスクールに通学中の方）：学年末は大変混雑する傾向があるため、その期間中はすべてのアセスメント依頼にお応えできない場合があります。また、長期休暇が始まると学校との連絡が取りにくくなります。包括的な評価を行うため、少なくとも2～3ヶ月前にはご予約をお願いいたします。例：学年末（6月）までに報告書が必要な場合は、4月上旬までにお申し込みください。

For Those Interested in Psychological Assessments：As it tends to get very busy at the end of the school year, we may not be able to accomodate all assessment requests during that time. Additionally, communication with schools becomes difficult once the long vacation begins.In order for us to provide a comprehensive assessment, we would like to ask that appointments are made at least two or three months in advance.Example: If you need the report by the end of the school year (June), please apply by early April.
